Multi-Sector Service Accountability Agreement

Stated as a matter of record for public viewing, this page contains a copy of the Service Accountablility Agreement between South West Local Health Integration Network (the "LHIN") - and - Addiction Services of Thames Valley (the "HSP").

Prior to providing funding for the provision of services to its local health system, the Local Health System Integration Act, 2006 requires that the LHIN and the HSP enter into a service accountablility agreement.

The service accountability agreement is a multi-year agreement. It supports a collaborative relationship between the LHIN and the HSP to improve the health of Ontarians through better access to high quality health services, to co-ordinate health care in local health systems and to manage the health system at the local level effectively and efficiently.

In this context, the HSP and the LHIN agree that the provision of services to the local health system by the HSP will be funded as set out in this Agreement.

The AGREEMENT became effective as of the 1'st day of April, 2011
