Program Eligibility & Admission Criteria
Primary Eligibility Requirements:
- Currently experiencing problematic substance use
- History of multiple entries in treatment continuum
- Homelessness - CURRENT AND HISTORICAL TRACK RECORD
- Cannot obtain or maintain housing without support
- Not experiencing a serious mental illness or a severe Acquired Brain Injury
- At risk of being homeless or marginally housed
Admission Criteria will include:
- Expression of a strong desire to live independently
- Must participate in a comprehensive screening & assessment process in order to develop an appropriate treatment plan
- Must participate in services that support stabilization
- Must agree to participate in the ASH Program including a treatment plan to change substance use patterns and practices (change includes harm reduction and abstinence goals as appropriate for individual) and express a strong desire to make changes in their problematic substance use
- Agreement to abide by rules, policies and procedures of program, including refraining from smoking inside the housing units
- Client must be willing to commit to their individualized treatment plan as developed in collaboration with their assigned Intensive Addictions Case Manager for a period up to 364 days
- Ability to participate in self-management of unit and basic home maintenance with assistance
- Ability to manage personal health care
- Legitimate income source
- Qualifies for rent supplement
- No impending jail terms (otherwise go on wait list)
- No imminent danger to self, staff or other participants

Referral Process:
- Clients must have a Standardized Provincial Assessment completed and a treatment plan that identifies housing as a barrier to stability . This may be done by attending ADSTV main office for Walk-In Intake to begin their process. If a client resides outside of London, Assessment groups are available in Middlesex, Oxford and Elgin County. Alternative arrangements may be made if these locations do not serve the needs of the client. If a client has recently had a Standardized Provincial Assessment completed those results should be faxed along with the referral. Assessment tools do not necessarily need to be current within the last 90 days at the time of the application.
Walk-In Intake 1:00pm: Monday Assessment groups begin at 2:00 and 3:00pm
Thursday Assessment group begins at 2:00pm - If clients are currently clients of ADSTV they should speak to their counselor about a referral otherwise, referrals may be made by a referring agency or the client.
- Once a referral is received, ASH staff will review and invite client to an Information Session or make arrangements to review material with clients.
- Clients will attend an Information Session and if client feels the program would be beneficial to them and they are able to commit to the ASH program they will be invited to attend an individual session with an Intensive Addictions Case Manager (IACM) with ASH
AND .. clients will meet individually with the Housing Provider in their area at an agreed upon time. At each of these individual sessions client will be asked to bring pertinent information to verify income and citizenship status (such as a birth certificate and a cheque stub from Ontario Works). - If client is appropriate the ASH team will present clients Application & Assessment to the Admissions Advisory Committee for review and prioritization. They will also have the opportunity to view an apartment to ensure they are making an informed choice about entering the program. All admission decisions are made by the Admissions Advisory Committee.
*Please note: If a client lives outside of London and/or would not be able to attend ADSTV main office due to travel restrictions or other concerns, alternate arrangements may be made when available.
